Adnexal mass — structural characterization with Doppler
Patient
Patient, female, 37 years old, chronic pelvic pain.
Clinical indication
Chronic pelvic pain for six months, unresponsive to initial clinical treatment.
Technique
Transvaginal ultrasound with color Doppler.
Ultrasound findings
Right adnexal formation, cystic, unilocular, with homogeneous ground-glass content, measuring 4.5 cm, without a solid component.
Doppler
No vascularity in a solid component, as none is present.
Differential diagnosis
Endometrioma · Resolving hemorrhagic cyst · Serous cystadenoma
Final diagnosis
Morphologic findings characteristic of an ovarian endometrioma.

- The ground-glass pattern is highly suggestive of chronic hemorrhagic content.
- Absence of a vascularized solid component significantly lowers the suspicion of malignancy.
