Differentiating a nodule from a granuloma changes management

Filler nodules tend to have regular, well-defined margins; foreign-body granulomas appear with indistinct borders and a more heterogeneous internal pattern. This distinction isn't academic — it determines whether management is conservative, an injection, or another approach.
