Liver elastography: when it changes management
Liver stiffness as a noninvasive substitute for part of the information that biopsy used to provide alone.
Shear wave liver elastography has become established as a method for staging fibrosis in chronic liver disease, reducing — without eliminating — the need for biopsy in a significant proportion of patients with hepatic steatosis.
What tissue stiffness represents
The propagation speed of the shear wave in the liver parenchyma correlates with the degree of fibrosis. Low values reliably rule out advanced fibrosis; very high values suggest established cirrhosis — the intermediate zone is where standalone interpretation loses strength and requires correlation with other markers.
Limitations worth remembering
Ascites, significant obesity, acute hepatic inflammation, and venous congestion can artificially raise the measured stiffness, generating false positives for advanced fibrosis. Interpreting the number without considering the clinical context is the most common mistake among beginner examiners.
